When you require hospitalization your regular physician will coordinate your admission with the Hospitalist team. Your Hospitalist will ensure that all medical history is received from your physician and that all information about your medical condition needed by your physician is available.  Your Hospitalist will also work with you and your family to make sure you understand your treatment throughout and even after your stay at Crossgates River Oaks Hospital.  

Your Hospitalist will:   
Assess and treat your medical condition throughout your stay at Crossgates River Oaks Hospital         

Coordinate your hospital care including diagnostic tests such as x-rays, blood tests or other procedures       
     
Arrange specialty care including therapy or consults with other specialists            
Transition your medical care back to your physician who will resume responsibility after discharge
